Eligibility

Referred by a physician, the patient or a family member.

Have a diagnosis of a terminal illness with a life expectancy of months rather than years.

Live in Ottawa County
Services Provided

Nursing Care by registered nurses.

Personal Care by Home Health Aides.

Grief and bereavement Counseling by Social Worker or Minister.
-
Visits from Social Worker to assist with emotional, psychosocial, feelings and other resources as needed.

Volunteers for family assistance and respite care.

Short Term Inpatient Care in Ottawa County Health Center.

Inpatient Respite Care.
